I only noticed when I connected power and the TL074 went boom! (it was more a loud click, kind of disappointing)

EDIT:
Looking at the pinout: can I simply rotate it 180°?? Sounds too good to be true!

This weekend i unhooked my wallwart from the surge protector. The shell surrounding the transformer came off POP.

Real glad my safety guidelines have the power strip in the off position when I leave it. That coulda been bad if there was mains running through it. Don’t think i touched anything when the cover slipped off, but it very well coulda happened.

I didnt get any pics, but it was like this:
